	Det Norske Veritas (DNV) Mohawk Valley Health System affiliates Faxton St. Luke’s Healthcare (FSLH) and St. Elizabeth Medical Center (SEMC) are accredited by Det Norske Veritas (DNV) under the National Integrated Accreditation for Healthcare Organizations (NIAHO) standards for compliance with Centers for Medicare and Medicaid Services (CMS) Conditions of Participation (COPs).
	American College of Cardiology (ACC) Mohawk Valley Health System (MVHS) is the first in New York State to receive accreditation of its cardiac catheterization by the American College of Cardiology (ACC). This accreditation recognizes MVHS for demonstrating expertise and commitment in treating patients who come to the Cardiac Cath Lab for care.
	Healthgrades Specialty Excellence Award – Surgical Care MVHS has been recognized with the Surgical Care Excellence Award, placing the hospital in the Top 10 percent in the nation for Surgical Care. For its analysis, Healthgrades evaluated approximately 45 million Medicare inpatient records for nearly 4,500 short-term acute care hospitals nationwide to assess hospital performance in 31 common conditions and procedures and evaluated outcomes in appendectomy and bariatric surgery using all-payer data provided by 16 states. Healthgrades recognizes a hospital’s quality achievements for cohort-specific performance, specialty area performance, and overall clinical quality. 	National Committee for Quality Assurance Patient-Centered Medical Home Recognition The National Committee for Quality Assurance (NCQA) recognizes the Adirondack Community Physicians medical offices for their Physician Practice Connections – Patient-Centered Medical Home (PPC-PCMH) program. The recognition notes their use of evidence-based, patient-centered processes that focus on highly coordinated care and long-term provider/patient relationships.
	ISO 9001:2015 Accreditation Mohawk Valley Health System affiliates Faxton St. Luke’s Healthcare (FSLH) and St. Elizabeth Medical Center are accredited by Det Norske Veritas’s (DNV) National Integrated Accreditation for Healthcare Organizations (NIAHO℠) and are certified to the ISO 9001:2015 Quality Management System Standard.Becoming ISO certified is an additional component of the DNV accreditation.
	HeartCARE Center: National Distinction of Excellence The American College of Cardiology established the HeartCARE Center: National Distinction of Excellence award in 2018 to recognize hospitals that demonstrate a commitment to comprehensive high-quality culture. 	Get With The Guidelines® – Stroke Gold Plus with Honor Roll The Stroke Center at the St. Luke’s Campus of MVHS is the area’s only designated Primary Stroke Center, received the American Heart Association/American Stroke Association’s Get With The Guidelines® – Stroke Gold Plus with Honor Roll quality achievement award for the 10th consecutive year. 	Excellus Blue Distinction Center+ for Cardiac Care Blue Distinction Centers and Blue Distinction Centers+ for Cardiac Care provide a full range of inpatient cardiac care services, including coronary artery bypass graft surgery, heart valve surgery, and angioplasty (percutaneous coronary intervention). 	Excellus Blue Distinction Center+ for Bariatric Surgery Blue Distinction Centers and Blue Distinction Centers+ for Bariatric Surgery provide a full range of bariatric surgery care; including surgical care, post-operative care, outpatient follow-up care and patient education. 	DNV Certification – Comprehensive Stroke Center MVHS has the highest level of treatment for serious stroke events and the St. Luke’s Campus is the best-equipped hospital in the area to diagnose and treat any kind of stroke or stroke complication. 	Commission on Cancer The Cancer Center is nationally accredited by the Commission on Cancer. 	Centers for Disease Control and Prevention’s (CDC) Diabetes Prevention Recognition Program (DPRP) Full Recognition The Centers for Disease Control and Prevention (CDC) established the CDC Diabetes Prevention Recognition Program (DPRP) as part of the National Diabetes Prevention Program. The DPRP provides information to people at high risk of type 2 diabetes, their health care providers, and health payers on the location and performance of local type 2 diabetes prevention programs. 	American College of Radiology (ACR) – Accredited Facility in Ultrasound The Ultrasound Accreditation Program involves the acquisition of clinical images, submission of relevant physician reports corresponding to clinical images submitted, and quality control documentation. 	American College of Radiology (ACR) – Accredited Facility in Stereotactic Breast Biopsy The Stereotactic Breast Biopsy Accreditation Program provides facilities performing stereotactic breast biopsy procedures with peer review and constructive feedback on their staff’s qualifications, equipment, quality control, quality assurance, accuracy of needle placement, image quality and dose. 	American College of Radiology (ACR) – Accredited Facility in Positron Emission Tomography The ACR Positron Emission Tomography (PET) Accreditation program was developed and is directed by the Committee on Nuclear Medicine Accreditation of the Commission on Quality and Safety. 	American College of Radiology (ACR) – Accredited Facility in Nuclear Medicine The Nuclear Medicine Accreditation Program involves the acquisition of clinical and phantom images and corresponding data for each unit. All providers that bill for nuclear medicine and PET under part B of the Medicare Physician Fee Schedule must be accredited in order to receive technical component reimbursement from Medicare. 	American College of Radiology (ACR) – Accredited Facility in Mammography The Mammography Accreditation Program provides facilities with peer review and constructive feedback on staff qualifications, equipment, quality control, quality assurance, image quality and radiation dose. 	American College of Radiology (ACR) – Accredited Facility in Computed Tomography The CT Accreditation Program involves the acquisition of clinical and phantom images, dose measurements and the submission of scanning protocols. Providers that bill under part B of the Medicare Physician Fee Schedule must be accredited in order to receive technical component reimbursement from Medicare. 	American College of Radiology (ACR) – Accredited Facility in Breast Ultrasound The Breast Ultrasound Accreditation Program provides facilities performing breast ultrasound and ultrasound-guided breast biopsies peer review and constructive feedback on their staff’s qualifications, equipment, quality control, quality assurance, accuracy of needle placement and image quality. 	American Academy of Sleep Medicine Accredited Member Center Each of these sleep centers has demonstrated a commitment to the highest quality of care in the treatment and diagnosis of sleep disorders. In order to become accredited by the American Academy of Sleep Medicine, sleep centers must comply with the AASM Standards for Accreditation, the gold standard for patient care in the sleep medicine field.
