The Faxton St. Luke’s Healthcare (FSLH) Retail Pharmacy provides in-house prescription services to all Mohawk Valley Health System (MVHS) employees and their dependents with Excellus BlueCross BlueShield health insurance coverage provided through MVHS.
The FSLH Retail Pharmacy is staffed by a pharmacist and technicians who have a thorough understanding of the Excellus health insurance plan, associated formularies and our specialty drug program.
From maintenance medications and mail orders to one-time prescriptions and over the counter offerings, the FSLH Retail Pharmacy is a convenient alternative to traditional drugstores!

The FSLH Retail Pharmacy is considered a domestic provider for non-union employees with MVHS insurance, which means that prescriptions filled here have a lower copay structure.

Location
The pharmacy is located on the first floor of the St. Luke’s Campus in the Professional Office Building next to Centrex Labs. Reserved parking for medication pick-up is available.
Eligibility
At this time, only employees and their dependents with Excellus BlueCross BlueShield health insurance overage can use the FSLH Retail Pharmacy. We hope to extend this benefit to all employees and their families in the future.

Prescription refills are easy and available through the MyChart App, but if you require assistance, please call 315-624-myRx(6979).
How to Transfer Prescriptions
Please complete the Prescription Refill Transfer Request Form and Patient Information Form (found on the MHVS intranet). You may fax completed forms to (315) 624-6650.
